The 2010 Summer Program Designed to Train Champions
The success of our Soccer Program doesn’t just happen; it takes the dedication and hard work of the players and coaches throughout the year, not just during the season. To aid in the development of our players and teams we have developed a summer program designed to “Train Champions”. The focus of the program is on developing the individual player’s ability to dominate their opponent on the field. The summer program also helps to build team and program unity, gives incoming players an opportunity to see what High School Soccer is all about. This year the program includes the Westlake Soccer Camp, Summer Training and a Boys Summer League.
June 14-18. Westlake Soccer CampOur Thirteenth annual camp, this year it will again be run by Coach Propster and Coach Marino. The camp is open to players age 13-18 and runs from 1:30-3:30 PM. Cost is $150. To register fill, out and mail in the attached Medical Release Form.
June 21 – July 9. Summer TrainingThis is one of the best parts of the summer program as it gives us a chance to really work with you to help you become the best you can be; to be able to dominate your opponent.Sessions are Monday, Wednesday and Friday and run from 1:30-3:30 PM.Cost is $150 and you must have turned in the attached Medical Release Form.
Boys Summer League Dates and times to be announced. The purpose of this league is to give the coaches an opportunity to work with younger players in a game situation. There is no fee but to participate you must attend the summer training.
Participation in the summer program is optional and there are fees involved, but we highly recommend you attend as much as possible. No WestlakeHigh School player will be excluded for lack of ability to pay. There are scholarships available. If you have any questions please call Coach Propster at 805-376-1903.

WAIVER AND CONSENT FORM
PARTICIPANT HOLD HARMLESS AGREEMENT
I (we) the parent/guardian of the above named player recognizing the possibility of physical injury associated with soccer and in consideration for Westlake Soccer accepting the player for participation in its programs and activities (the “Programs”) hereby give my (our) consent for emergency medical care for any or all injuries sustained while attending any of the Programs’ activities.This care may be given under whatever conditions are necessary to preserve the life, limb or well being of my dependent.I understand this may include transportation and treatment at a local hospital.
In consideration of your permitting the player to participate in the Westlake Soccer Program and in consideration of the time and effort devoted by the coaches, supervisors and staff of the Program, I (we) assume all risks and hazards incidental to the conduct of the activities of the Programs and transportation to and from the activities.
I (we) further hereby release, waive, hold harmless, discharge and otherwise indemnify the Westlake Soccer Program, Westlake Soccer Camp, Westlake High School, the coaches and staff of the Program, and the Westlake High School Soccer Booster Club from any and all liability and /or responsibility and any claim by or on behalf of the player as a result of the player’s participation in the activities of the Programs and or transportation to or from the same, which transportation I (we) hereby authorize.

Dear Warrior Soccer Families:
We’ve held a number of fundraisers this year and would like to thank you all for your participation! It’s hard to believe the season is quickly coming to an end, and with that in mind we ask that each player’s family consider supporting the Westlake Warrior 2009 -2010 Soccer Yearbook.
This book is a new addition for our program, and if supported properly can become an annual item for us. Along with the various squad and individual pictures, we plan on having plenty of photos of the boys and girls at practice, scrimmages and games. We would like it to be not only informational but to be cherished as a keepsake for every Warrior soccer player.
The Yearbook is not a fundraiser. Ads and dedication pages in the book simply cover the costs of printing and production. The more ads we sell, the more pages we can devote to images of the players and the bigger and better the book will be!

Please consider a contribution toward the ad placements in the yearbook, whether by your own business or businesses your frequent – hard stylist to foot doctor – or a dedication from parents or other family well-wishers.
Ads will be printed in color and should be submitted in electronic format, via email. Camera-ready artwork or high quality printed material that can be scanned will be accepted if in good condition – no smudges, staples or wrinkles, as these imperfections will show up on the scanned version.
Make certain that you have a Yearbook Ad / Dedication Registration Form (attached) filled out for each one of the ads you sell and that all information is completed and legible so that we can contact the purchaser if necessary. Make as many copies as you need and turn in the completed form with each ad and check you collect.
